Social Services
Emotional stresses can interfere with the effectiveness of medical treatments. Social Services helps patients overcome those stresses and assists in the development of positive attitudes that will contribute to the recovery process.
The Social Services staff provides special assistance through professional counseling and discharge planning that focuses on the patient's needs after hospitalization.
Family, friends, hospital staff, physicians and community agencies may make referrals for social services. Services provided include these and more:
- Referrals to community resources, financial assistance referrals and equipment acquisition
- Coordination of services for the patient
- Individual, group and family counseling
- Follow-up services for patients and families
- Psychosocial assessment and evaluations
- Coordination of placement with family and placement facility for extended care
